So just a few thoughts off the top of my head...

1. For Music on Hold devices like this, you have to have some sort of isolation between the device and the PBX. So typically Avaya has used a 909A/B Coupler, and the newer G700/350/250/430/450 required an older KS-23395 Couplers (Which yes it says for System 25 but it works all the same).

2. Instead of using that MOH Device, do you have VAL Boards? You can place a .wav file onto a VAL and then use it on your system as a music on hold. Just a thought.
